CentraCare Family Health Center
CentraCare Family Health Center, formerly known as Mid-Minnesota Family Medicine Center, is the clinic
of the University of Minnesota/St. Cloud Hospital Family Medicine Residency program. As the only
residency program in Central Minnesota, the goal is
to train competent, caring family physicians to
practice in rural communities.
Our mission is to provide
primary care treatment,
prevention, and education to all
patients, including those who
are underserved or
underinsured, while training
competent and caring family
physicians to practice in rural
communities.
As a patient of a teaching clinic, you will benefit
from cutting edge medical technology while
receiving services similar to other clinics.
Appointments are with medical doctors who have
already graduated with their medical degree and are
completing their three-year residency. They are
under the guidance of knowledgeable faculty
doctors.

- Accepts insured and uninsured patients assuring
continuity of care
- First clinic in the area to utilize the Electronic
Medical Record (EMR)
- No one is denied care because of inability to
pay
- Online appointment requests
- Unique services such as sliding fee scale, access
nurse and advocacy services and free
interpreter services
- Lab, X-ray and pharmacy services available in
the same building
The clinic is located in the same building as CentraCare Clinic - Northway Family Medicine.
Scheduling
An appointment must be scheduled for each family
member that needs to be seen. If interpreting
services are needed, please let us know when you
schedule your appointment. We prefer family members do not
interpret for you.
Insurance
You will need to bring your insurance information
with you at your first visit. Charges for services can
be paid by insurance (Medicare, Medicaid,
MinnesotaCare, private insurance) or by the
individual. Most insurances accepted.
Community Partnerships
CentraCare Family Health Center is a strong
medical resource in our region. As part of our
commitment to the community we partner with
numerous agencies, schools and programs to offer
these programs:
- Senior linkage prescription drug assistance
- SAGE Minnesota Cancer Screening Program to
increase breast and cervical cancer screenings
- Tri-County Action Program (TRI-CAP) to assist
low-income residents in Benton, Sherburne and
Stearns counties
- Partner with many schools and colleges in
Central Minnesota to train nursing and
pharmacy students
