CentraCare Clinic - 2010 Quality Award

It is time to celebrate Quality!
Each year the Quality Improvement department solicits nominations for the annual Quality Award. Staff was encouraged to nominate improvement initiatives that have positively impacted the care received by our patients. We are proud to announce the 2010 Quality Award to the Long Prairie for their work with DIAMOND and Health Care Home implementation.

2009: CentraCare Clinic Business Office staff for their participation in the PQRI program and meeting the reporting goals set by CMS.
2008: CentraCare Clinic – Melrose for their work with the DIAMOND Initiative and CentraCare Clinic Becker for their work in Diabetes Improvement.
2007: CentraCare Clinic - Melrose for their work in depression care.
2006: CentraCare Clinic - Heartland Family Medicine Clinic for their work implementing the Electronic Medical Record.
2005: CentraCare Clinic - Long Prairie for their work in diabetes care.
