CentraCare Clinic - 2008 Quality Award

It is time to celebrate Quality!
Each year the Quality Improvement department solicits nominations for the annual Quality Award. Staff were encouraged to nominate improvement initiatives that have positively impacted the care received by our patients. We are proud to announce that this year we have a tie and are awarding the 2008 Quality Award to two recipients.

Staff of CentraCare Clinic - Melrose

CentraCare Clinic - Melrose: DIAMOND Initiative Building on previous work that earned the Melrose site the Quality Award in 2007, the team in Melrose worked collaboratively with ICSI to become a DIAMOND-certified clinic in Minnesota. Implementing a care manager model which is reimbursed by major health plans has resulted in a system redesign that enhances the care for patients with Major Depressive Disorder. Data collected from the beginning of the program has demonstrated improved outcomes for patients.

Staff of CentraCare Clinic - Becker

CentraCare Clinic - Becker: Diabetes Improvement Spurred into action by less than optimal diabetes results, the Becker site developed a team to focus on improving diabetes care. Their teams established goals and by engaging staff at all levels, were able to achieve goals which originally appeared out of reach. Their hard work has resulted in better patient care demonstrated by improved patient outcomes. The Becker site has become the leading clinic in diabetes care for CentraCare Clinic.

Blue Cross Blue Shield Recognizing Excellence Program

Blue Cross and Blue Shield’s Recognizing Excellence incentive program recognizes providers for superior outcomes. The program established clinical outcome goals in five practice specialties which included: family practice, internal medicine, ob-gyn, pediatrics and cardiology.
