Grant Application Guidelines
Mission Statement:
CentraCare Health Foundation engages the philanthropic community in partnership to improve health and health care in Central Minnesota. We enable life victories everyday.
In pursuit of this mission, the Foundation will fund or otherwise sponsor:
- Health education programs
- Research to improve health
- Programs that improve health services
The Foundation will fund organizational, or institutional proposals that will support these objectives and lead to improvement in the health of Central Minnesota.
General Guidelines:
The Foundation Board of Directors, in evaluating grant applications, will be guided by the following general principles:
- Proposals must be consistent with the Mission Statement of the CentraCare Health Foundation.
- Grants will be made to projects which will have primary impact in the following Central Minnesota counties: Benton, Crow Wing, Douglas, Kandiyohi, Meeker, Mille Lacs, Morrison, Pope, Sherburne, Stearns, Todd, Wadena and Wright. View service area map
- The Foundation is reluctant to provide funding for: ongoing operational support or general administrative expenses for existing programs. Proposals that address health issues through partnerships with multiple individuals and organizations are encouraged.
- All decisions regarding funding will be made without regard to race, color, creed, sex, handicap, religion, or national origin of the grant seeker, and without regard to an applicant’s relationship with the CentraCare System or its subsidiaries.
Application Procedure:
- Grants will be awarded four times per calendar year by the Board of Directors.
- An applicant should contact the CentraCare Foundation office with a brief note of inquiry, describing the organization, the proposed project, an outline of the plan, and a general statement as to how the project will meet the goals of the Foundation.
- An administrative determination will be made as to whether or not the applicant is consistent with the mission of the Foundation and if so, you will be asked to make a formal grant application.
- Grant applications must be received by the published grant deadline to be considered for funding. Please provide 12 copies of your application – do not bind or staple. See deadlines below
- Projects of a technical or scientific nature will be referred to a physician board member for evaluation and recommendation, or for further referral if appropriate.
- At the meeting of the Board of Directors, an administrative report and recommendation will be made and, if appropriate, a technical report will also be given.
- Notification will be made following the Board decision.
- If a grant application is approved, arrangements for payment and other details will be arranged by the grant recipient with the Foundation’s administrative office.
- Progress reports will be required on at least a semi-annual basis, with a final report required at the conclusion of the project.
- In the event a grant application is accepted, and the applicant is notified but the applicant makes no request for the funds or for the deferral of the project in the succeeding 12 months, then the applicant must resubmit a new proposal for consideration.
- If an accepted project is to have two separate yearly grants, the second allocation will not be made without adequate reporting of the first year’s activity and the use of grant moneys.
Contents of Application:
A formal grant application should provide the following information:
- The name of the applicant, address, telephone number, name of person preparing the application, and the name of the person responsible for carrying out the project.
- A brief history of the organization or the background and relevant experience of an individual applicant.
- A detailed description of the proposed project with a needs assessment, an outline of the project goals and objectives, and the plan for meeting those goals and objectives.
- For research projects, or where otherwise appropriate, evidence of literature search using generally accepted research methodology.
- The geographic and population scope of the project.
- A description of the anticipated impact the project will have on the region.
- A description of how the program does not duplicate existing programs, or what distinguishes the project from programs already in existence.
- List individuals or organizations that are partners with your project.
- The time period to be covered by the grant.
- The project budget, including sources of income and anticipated costs. The Grants Committee may request an annual audit, if appropriate.
- Anticipated sources of income and budget for three years following the Foundation grant, if appropriate.
