The nursing home facility of Long Prairie Memorial Hospital and Home provides a caring, competent staff and a warm, home-like environment for our residents. We strive to provide the highest level of care possible—always keeping in mind the needs of our residents and their families. We are dedicated to making our facility safe and secure for all residents.

We provide 24-hour, skilled nursing care by nurses who are specially trained in geriatrics and/or rehabilitation. In addition to our nursing staff, our care team includes a physician, a physical therapist and a social worker. A psychologist is available to see patients and review behavior care plans, and a psychiatrist is available for consultation as needed.

The design of the nursing home features a “neighborhood concept,” which means that each wing, or neighborhood, has a distinct, home-like atmosphere and dedicated care team.

Long Prairie Memorial Hospital and Home is licensed by the Minnesota Department of Health.

Rose Lane
Rose Lane is a unit within the nursing home designated for residents with certain memory loss and behavior conditions. Specially trained nurses and staff care for Rose Lane residents 24 hours a day. This unit has been carefully designed to decrease stimulation and ensure residents’ safety and security. Special programming is offered, and safety measures are taken to care for residents who may wander or exhibit other memory loss conditions.

Eden Alternatives
To meet our goal of providing resident-centered care, we provide the Eden Alternatives program. The philosophy of this program is to involve residents in making decisions about their lifestyle and care. Greater input from residents helps the facility become more home-like for those who live here. The program also helps meet some of the social needs of our residents.

Our numerous green plants and our pet dogs, cats and birds are just a few features of the Eden Alternatives program. Additionally, we encourage visits from children to further enhance the home-like quality of our facility.

Fitness for Fun Program
As the name suggests, this program focuses on both fitness and fun. Fitness classes are held in our community activity room several days a week. The class instructor adapts exercises to meet a variety of skill levels and physical conditions.

Adult Day Services
This program offers companionship and a wide range of social and cultural activities for area seniors. The program is offered weekdays from 8 a.m. to 4:30 p.m. at Long Prairie Memorial Nursing Home. A noon meal and snacks are provided.
