Home Care Services

St. Cloud Hospital Home Care is a comprehensive home care agency developed to meet the needs of patients who are best served at home. Patients qualifying for home health care are those who require extended professional care after discharge from the hospital or during the course of clinic treatment. Our dedicated staff of professionals is sensitive to the needs of patients and their families. We provide high-quality medical care and emotional support while encouraging independent living.

Home care is provided seven days a week. A registered nurse is on-call 24 hours a day. Self-referrals for home care services are accepted or a referral may be made by a physician, family member or friend. Once the referral is made, our staff will contact the patient’s physician to discuss the services needed. A nurse or therapist will provide an initial assessment to the patient.

Most insurers cover skilled home care services. Home care charges will be billed to you and submitted to your insurance company (Medicare, Medicaid and/or private insurance).

Through our extended care program, we provide services for patients who are medically stable, but still need some assistance at home. These services are usually paid for by the client, though extended care services may be covered by Medicare or long-term care insurance.

Palliative care is a new program that assists patients and families in dealing with advanced illness. The program aims to alleviate pain and provide social, spiritual and emotional support, regardless of whether the patient chooses curative or comfort care. Patients may still receive treatment, such as dialysis, radiation and transfusions.

Staff
A team of skilled professionals works together with the patient’s physician to provide a total continuum of care. Our home care team includes:

  • registered nurses
  • licensed practical nurses
  • physical and occupational therapists
  • speech language pathologists
  • medical social workers
  • home health aides

Area Served
St. Cloud Hospital Home Care Services provides care for those living within a 35-mile radius of St. Cloud Hospital. In addition, we have an office location at Park View Center in Melrose.

Accreditation
St. Cloud Hospital’s Home Care program is licensed by the state of Minnesota, certified by Medicare and accredited by the Joint Commission on Accreditation of Health Care Organizations. These regulatory agencies provide assurance that you will receive the highest quality care available.
