| When might Home Care Services be needed? Any of the following might indicate a need for home care services:
- Frequent emergency room visits
- Post-surgical patients
- New diagnosis or flare up of a chronic disease
- Multiple illnesses
- Lives alone
- Pain control issues
- Caregiver concerns
- New medications or multiple medications
- Home safety concerns
- Older population (although we also serve pediatrics)
- Assessments for a smooth transition to the home setting
Services Offered at Home
Home care can be the bridge that helps keep patients safe at home.
Services include:
- Patient education related to new a diagnosis and care at home
- Monitoring of incisions for signs and symptoms of infection
- Wound care
- Pain control issues/pain medications
- Multiple medications/medication set up
- Home exercise programs
- Home safety concerns
- Home adaptations
- Blood draws
- Home infusion
- Telemonitoring
- Care of infants and children
- Care of seriously ill patients
- Dementia patients
- Transition from a nursing home to the home setting
- Caregiver needs assistance at home
Home care visits can be provided seven days a week. A registered nurse is on-call 24 hours a day.
Referrals
Self-referrals for home care services are accepted or a referral may be made by a physician, family member or friend. Once the referral is made, our staff will contact the patient’s physician to discuss the services needed. A nurse or therapist will provide a free initial assessment of the patient in their home.
