What is MyChart?
MyChart is a secure Internet access to an electronic version of your medical record. MyChart is designed to enhance your medical care by giving you better access to your medical record information. MyChart contains selected, limited medical information from your medical record and does not reflect the complete contents of your medical record.
Which CentraCare Clinics offer MyChart?
CentraCare is currently piloting MyChart at CentraCare Clinic – Becker. CentraCare Clinic will continue to expand MyChart access to other clinics.
Who can sign up for MyChart?
You must be at least 14 years old to sign up for MyChart. CentraCare is in the process of making MyChart available to all of its clinics. You may sign up for MyChart once it is available at your clinic.
Is MyChart Secure?
Yes. MyChart is a secure Internet site and provides encrypted communication in full compliance with state and federal requirements. Access is granted only to users who have registered with their clinic and have completed an authorization form. Access to your health information in MyChart is secured by your personalized ID and password, known only to you.
What do I do with the access code and instructions?
Your access code is a unique identifier you will use -- along with the last 4 digits of your social security number and your date of birth -- to log into MyChart for the first time. The first time you log in, you will click on the “New User” link and follow the instructions to enter your access code. (These codes require correct use of upper and lower case letters, so please enter them accurately. The access code will expire 30 days from the time it is generated.)
How do I choose my personal MyChart ID and password?
During your first log-in, you will be prompted to create a log-in ID and password. This is the ID you will use every time you log-in to MyChart. Once selected, your MyChart ID cannot be changed. Choose an ID and password that are unique to you and easy for you to remember. Avoid using an ID or password that are easy for others to guess, such as your first or last name.
- Your ID must be between 6 and 18 characters long. Only letters or numbers are allowed and it cannot contain any spaces.
- Your password must be between 6 and 18 characters long and must include at least one number and one letter.
- You will need to enter an e-mail address so that MyChart can notify you when you have new information to view and also to allow for fast and simple password reset in the case you happen to forget your password.
How does the password reset question and answer work?
The password reset question and answer set will allow you to reset your password if you forget it, or allow you to change your password at any time you wish. When you log-in the first time and setup your personal MyChart ID and password, you can also specify a password reset question and matching answer. Common password reset questions are: “What is your mother’s maiden name?,” “What is your pet’s name?,” “What is your favorite color?” First, you will enter your password reset question, then you will enter the correct answer to the question. In the future, if you have forgotten your password or simply wish to change it, you can click on the “Forgot Password?” link on the log-in page. The system will ask you the password reset question. If you correctly answer this question, you will be allowed to reset your password. (Please keep in mind that your password reset answer cannot be the same as your MyChart password.)
What if I forget my MyChart ID?
Your MyChart ID is permanent and cannot be changed. If you forget your MyChart ID, please contact the MyChart representative at your primary care clinic. You will be given a new access code to use to sign in to MyChart. Once you have entered your new access code, along with your social security number and date of birth, your original MyChart ID will appear. You will then be asked to select a new password.
What if I forget my password?
If you forget your password, you can set up a new one by clicking on the “Forgot Password?” link on the log-in page. Follow the instructions, correctly answer your password reset question, and you will be allowed to reset your password.
May I access my children’s medical records?
Yes. You are granted full authority by proxy to access your child’s medical chart if your child is between the ages of 0-12. If your child is between the ages of 13-18 you will be granted partial access to his/her medical record. These age ranges comply with state regulations designed to protect minors who seek treatment for pregnancy, chemical abuse and sexually transmitted diseases. For further information on access to your child’s medical records please contact the MyChart representative at your primary care clinic.
I once had access to my child’s MyChart information and now I do not. What happened?
If your child recently turned 13 years old, you must reapply for access to his or her MyChart information. To protect the privacy and confidentiality of health information according to state and federal laws, CentraCare is not able to provide online access to the medical information of any child 13 years or older. You only will be able to use MyChart to request appointments for your child and view and print his or her immunization record. If your child has recently turned 18 years old, CentraCare can no longer provide you access to his or her MyChart information.
I once had access to another adult’s MyChart information and now I do not. What happened?
Access to another adult’s MyChart information expires after one year. If you want to regain access, the adult listed on the MyChart account must complete and resubmit the MyChart Proxy Authorization Form.
May I give others access to my records?
Yes. You can grant a spouse, or someone who helps you manage your health, full access to your medical records. Ask your clinic’s MyChart representative for more information.
Will there be any charges for using MyChart?
Currently, there are no fees to use MyChart. However, some insurance companies are preparing to offer online services, like E-visits, through programs like MyChart. An E-visit will be treated like a regular office visit and your insurance company would be billed for it as such. In addition, your insurance company may require that you pay a co-pay for E-visits. Check with your insurance company to see if they have plans to offer E-visits through programs like MyChart.
Minimum Computer Requirements
To access MyChart, you will need:
1. A personal computer
2. An Internet connection with web browser
3. A personal e-mail address
Suggested Screen Resolution
MyChart screens are best viewed with your screen resolution setting at 800x600 pixels or higher. If your screen resolution is less, you will see larger scroll-bars at the right side and bottom of the screen.
Suggested Web Browsers
Internet Explorer, Firefox, Safari or Netscape are recommended. Difficulties are known to occur when other Internet browsers are used. You may download Netscape and Mozilla Firefox browsers free of charge at www.netscape.com or www.mozilla.org.
