MyChart Sign-Up and Proxy Form - Use this form to request an activation code which will allow you to create a MyChart account to view your MyChart information. You may optionally request proxy access to your children’s MyChart information or access another adult’s MyChart information by completing the appropriate proxy information on the form. Simply print and mail to the address listed on the form.
