What is a Health Care Directive?

A Health Care Directive is a written document that informs others about your health care wishes if you are unable to speak for yourself.

Complete Your Form Online

  • Fillable online forms and guides on how to complete the forms are available in English and several additional languages.

Once you have completed the health care directive form, remember the following:

  • Your signature on the document needs to be witnessed or notarized.
  • Make copies of the document for yourself and your loved ones.
  • Submit the document to your physician for your medical record.
