Student/Resident Rotations: MD, DO, DPM, CNM, APP/APRN/DNP, CRNA & GC Programs

If you are in a residency program or are enrolled in a MD, DO, DPM, CNM, APP/APRN/DNP, CRNA or GC program and are interested in a rotation experience at a CentraCare location, please carefully review and complete each of the following sections.

Please note that all requests for rotations must go through CentraCare. Solicitation of providers is prohibited.

All CentraCare staff, providers, volunteers, contract workers and students are required to be vaccinated for COVID-19. A religious and medical exemption application are available upon request.

Applicant Instructions

Step 1:

Please complete the Application to CentraCare for Student/Resident Rotation Experience (PDF) and sign pages 3 and 4. For the best user experience, download the application to your computer prior to completing.

Step 2:

Submit your application to the program representative at your school along with all the required documentation specified on the application. To ensure all requirements are provided upon submission, a checklist is available following the application.

Once your program representative has received your application in entirety, he/she will submit it to CentraCare on your behalf. All communication regarding possible rotations or questions must be between the school and CentraCare at MedStudents@centracare.com.

Program Representative Instructions

Step 1:

Upon receipt of the applicant’s documents, ensure all applicant requirements have been provided. An applicant submission checklist is available following the application.

Step 2:

Complete the program representative submission checklist which can be found following the applicant’s checklist and sign pages 4 and 5.

Step 3:

Submit all materials in one PDF file to MedStudents@centracare.com. Applications with missing documentation will not be considered.

  1. CentraCare has elected to focus on those programs with a campus component or hybrid format (online plus on campus) and will not be partnering with solely online programs or providing rotations to students who enroll in exclusively online programs.
  2. Rotation requests from APP applicants who are not currently employed by CentraCare will be considered starting 3 months prior to the requested start date.
  3. Please Note: If an APP student is not a current or previous CentraCare employee, does not have a connection to Minnesota, is in a non-Minnesota program or is not interested in future employment at CentraCare — a rotation is unlikely due to the high volume of requests.
  4. All application materials must be received to MedStudents@centracare.com at least 6 weeks prior to the student’s rotation start date.
  5. Submission of required application and documentation material does not mean a rotation has been approved.
  6. A CentraCare student facilitator will notify the program representative if the requested rotation has been approved or if unable to accommodate a rotation experience.
  7. Applications with missing documentation will not be considered.
  8. Please allow up to three weeks to receive a response pertaining to rotation availability.
  9. If a requested rotation is no longer needed, the CentraCare student facilitator must be immediately notified.

Prior to the start of an approved rotation with CentraCare, the following information must be completed and/or on file with CentraCare.

  1. Current Affiliation Agreement between the school/program and CentraCare. CentraCare will initiate this agreement.
  2. After all paperwork has been received and a rotation has been approved, mandatory online education/orientation modules must be completed. CentraCare will provide the applicant with a User ID and password, and the modules can be completed off site. Education/orientation must be completed a minimum of 2 weeks before the start of the rotation and may take 2-4 hours to complete.
  3. An Epic trainer will contact the applicant prior to the rotation to schedule onsite Epic training (EMR).
