Get Help with Advance Care Planning
Do you need help filling out your health care directive? Free educational sessions are offered in-person at Whitney Senior Center in St. Cloud at 11 a.m. on the first and third Tuesdays each month.
These free sessions are offered to community members. Get all your questions answered about health care directives and Advance Care Planning. Open to anyone over the age of 18.
Advance Care Planning representatives will be available to provide access to forms and information, answer questions, and assist in properly completing your health care directive. Leave specific instructions about your medical choices in the event you are no longer able to make decisions for yourself.
