Get Help with Advance Care Planning
Do you need help filling out your health care directive? Free educational sessions are offered in-person or virtually through Honoring Choices.
These free sessions are offered to community members in various locations. Virtual sessions are also available via Zoom (day or evening). Get all your questions answered about health care directives and Advance Care Planning. Open to anyone over the age of 18.
Certified Advance Care Planning representatives will be available to provide access to forms and information, answer questions, and assist in properly completing your health care directive. Leave specific instructions about your medical choices in the event you are no longer able to make decisions for yourself.
