Earning Points
Earn 1 point for every $1 you spend on:
- Botox
- Injectables, including dermal fillers, Kybella, Sculptra and SKINVIVE
- Skin care products and services
Restrictions on earning points:
- Points are earned on purchase price only. Points are not earned on sales tax, shipping or handling costs for products purchased or on any current or future taxes imposed in providing healthcare services.
- In the event of a product exchange or credit, the points are deducted from your Premier Loyalty Program account.
- Reward points cannot be earned from consultations, surgery or laser treatments with a Midsota surgeon or physician assistant, payment on an existing account, insurance copays or deductibles, facility or anesthesia fees, or on gift cards.
Redeeming Points
- Once you earn 500 points, you can redeem them.
- We will reduce your purchase by $25 for every 500 points you redeem.
- You may use points to purchase:
- Botox
- Fillers
- Kybella
- MedSpa services
- MedSpa products
- SKINVIVE
- Sculptra
- Points do not expire.
Restrictions on redeeming points:
- You may redeem points in increments of 500 only.
- Points redeemed may not apply toward insurance co-pays or deductibles, facility or anesthesia fees, or on gift cards.
- Points redeemed may not apply toward payment on an existing account, except for the products and services listed above.
- Points may not be redeemed for cash. Points have no cash value.
- Points are non-transferable to another individual.
- CentraCare – Midsota Plastic Surgery may, at its discretion, terminate or change the Premier Loyalty Program at any time.
