As of January 2, 2021, CentraCare and Carris Health does not participate in the Humana Medicare Advantage program. If you elect this plan, you can still be cared for by CentraCare, but we will be an out-of-network provider. This means your out-of-pocket costs may be higher, depending on services received.
UCare is exiting the Minnesota Medicare Advantage market effective Dec. 31, 2025. As a result, UCare is no longer an option for consumers to choose for the 2026 Medicare Advantage benefit year. CentraCare remains in network and will participate with UCare’s Medicare Advantage network offerings through 2025.

Keep Your Information Current to Help Prevent Claim Denials and Unexpected Bills
To help us bill your insurance correctly and avoid delays in processing your claims, please review and update your insurance information regularly with CentraCare.
- If you have changed insurance plans, received a new insurance card or have multiple insurance policies, please provide the updated details to our office before your next visit.
- If you’re covered under more than one plan (for example, through both you and your spouse’s employer), please contact your insurance company to confirm which plan is primary and which is secondary. This process is called Coordination of Benefits (COB).
- If your coverage has recently changed, please notify our office as soon as possible and bring your new insurance card to your next appointment.
