Preregistration & Birth Certificate Forms

Preregister with the CentraCare location where you plan to deliver to prepare for your baby's arrival. Doing this early will help you feel ready as the months go by.

Preregister Online

Preregister for delivery as soon as possible, but no later than your 28th week of pregnancy. This simplifies the process when you arrive and ensures insurance information is on file. Preregistering guarantees we have your information and allows for quicker care in case of early or unexpected labor.

You can easily preregister for all CentraCare birthing locations online:

  1. Go to MyChart
  2. Click on Menu
  3. Choose "Register for My Delivery"

Contact Your Health Insurance Provider

Before delivery, you should also call your health insurance company to ask them:

  • What you need to do before coming to the hospital
  • How long you can stay after giving birth
  • Any other questions you have about your coverage

Preregister By Mail

If you cannot complete the form online, you can print it, complete it and mail it to where you plan to give birth.

Pregnancy & Birth App Registration

Get pregnancy, birth, breastfeeding and parenting information and resources by registering for our free Bump2Baby360 app.

Once you complete the registration form, you'll receive an email from noreply@bump2baby360.com containing a link, your username and your temporary password. Don't see the email in your inbox? Check your junk/spam folder.

When you log in for the first time, you must create a new password. After logging in, you can access Bump2Baby360 on any device with an internet connection. You may also download Bump2Baby360 from the Android or iPhone app store. Then, log into the app using the same username and password.
