Prepare For Your First Visit
We want your visit to be safe, comfortable and as pleasant as possible.
What to bring:
- Health history form [pdf] and list of medications, herbs and vitamin supplements you are taking or complete the questionnaire in MyChart
- Your schedule for planning future appointments
- Current insurance card(s) and photo ID
- X-ray films or CDs of scans related to your visit that were not completed by CentraCare
- A list of questions you have for your provider
- Music, reading or other activity you can do quietly for a few hours
- Family member or friend to offer support and/or drive you home
Please leave valuables like money, credit cards, and jewelry at home.
For your convenience, we provide TV and iPads with internet access. Pagers are available for patients so that they can explore our campus without worrying that they will miss their time.
If it has been more than 6 months, please complete a dental examination and cleaning. If your dentist recommends major dental work, please first discuss it with your cancer center physician.
Health Care Directive & Advance Care Planning
Who will speak for you if you can't speak for yourself?
A health care directive is a planning tool used to identify who you want to make health care decisions for you if you aren’t able to communicate. It puts your wishes regarding end-of-life care in writing. Forms also are available in Somali and Spanish.
