CentraCare Hospice offers an opportunity to embody the spirit of your loved one by including his or her name in our Book of Life. We are proud to present the 2025 Book of Life for year-round viewing.
If you would like to add your loved one’s name to the 2026 Book of Life, please email the name to foundation@centracare.com and include “Book of Life” in the subject line. Or, you can submit your loved one's name along with a gift to benefit our Hospice Program.
Names represented in Book of Life 2025 were collected as of Nov. 1, 2025.
