Michael Akers, PharmD, Transitions of Care Pharmacist

Michael Akers

I provide clinical pharmacy services focused on transitions of care. I also oversee a five-week patient care APPE rotation and serve as a primary preceptor for the University of Minnesota College of Pharmacy PGY1 ambulatory care residency program.

Teaching and mentoring are a meaningful part of my work. In 2016, I received the Preceptor of the Year Award, (Parliament of Distinction Award) from University of Minnesota College of Pharmacy for his preceptorship of residents within the PGY1 Ambulatory Care residency program. In 2017, I received the Preceptor of the Year award from the University of Minnesota College of Pharmacy for his preceptorship of APPE students. And in 2022, I received the Preceptor of the Year award for his work with the PGY1 residents at CentraCare.

Outside of work, I enjoy spending time with my family, hunting, fishing and taking on DIY projects at home.

Jordan Banister, PharmD, CDCES, Ambulatory Care Pharmacist

Jordan Banister

I am a clinical ambulatory pharmacist who provides Medication Therapy Management (MTM) services at CentraCare. I earned my Doctor of Pharmacy (PharmD) from North Dakota State University in 2015 and Certified Diabetes Care and Education Specialist (CDCES) certification in 2024.

When meeting with patients, I focus on clear communication, practical solutions, and building strong, supportive relationships. I value partnering with individuals on their journey toward better health and empowering them to take an active role in their care.

Outside of work, I love spending time with my husband, two daughters and our dog. I also enjoy Reformer Pilates, reading and shopping.

As a pharmacist with a background in Public Health, my main goal is to provide quality, patient focused care to help improve patient outcomes and overall health. I strive to use my knowledge and experience to ensure that my patients have a therapy plan that is both effective and affordable.

Outside of work, I enjoy traveling, being outdoors, and spending time with my family and friends.

Jenna Evenson

I grew up in St. Cloud and graduated from Tech High School in 2003 (go Tigers!). I graduated from the University of Minnesota College of Pharmacy in 2014. I enjoy helping people reach their health goals and supporting them on their health journey.

In my free time, I enjoy spending time with my cat, reading, building Lego sets, playing games, and being with friends and family.

Medication Therapy Management is a passion of mine. I enjoy partnering with patients and their health care teams to improve drug therapy outcomes and quality of life. My goal is to optimize the patient's medications by ensuring they are appropriate, effective, safe and affordable.

Outside of work, I enjoy spending time with my husband, family and friends. On my days off, you can find me drinking coffee, reading books, at the lake or shopping.

I enjoy working with patients who have complex medical needs and helping them find ways to simplify and improve their care. I have experience in most primary care areas and have a special interest in diabetes management. Building trustworthy relationships with my patients is a meaningful part of my work.

Outside of work, I enjoy spending time outdoors, fishing, reading and being with my dogs, Tank and Bonnie.

Jennifer Hiemenz, PharmD, Ambulatory Care Pharmacist

Jennifer Hiemenz

I provide compassionate, patient-centered care through comprehensive medication management for individuals living with chronic pain. I partner closely with patients and the interdisciplinary care team to optimize therapy, improve safety and enhance quality of life. My approach focuses on evidence-based strategies and personalized treatment plans that support overall well-being while minimizing risks.

In addition to my clinical work, I serve as the Residency Program Director for the CentraCare PGY1 Pharmacy Residency Program. I oversee the learning experiences and professional growth of newly licensed pharmacists, fostering skills in ambulatory care and preparing them to make a meaningful impact on rural health.

As a pharmacist at CentraCare – Paynesville, I truly enjoy supporting patients and providers in our rural service area with their medication-related questions, concerns and guidance in both the hospital and ambulatory care settings. Building personal relationships with our patients and community drives my passion to ensure we are meeting optimal health goals and individualizing medication use. I also enjoy working with pharmacy students and residents as a primary preceptor.

My family—my husband and our two children—is my “why” and a constant source of motivation.

My husband and I are proud parents of three boys ages 6 to 21. We enjoy traveling, driveway basketball, slow Saturdays and game nights. Our golden labradoodle, Graham, is always eager to be part of the fun whenever he can.

My goal on a daily basis is to ensure that patients feel heard and confident in their medication plan. I recognize that each patient possesses valuable, intuitive insight into their own health and I strive to honor that perspective in every interaction."

In my role as an ambulatory care pharmacist, I enjoy partnering with patients and providers to optimize medication therapy and improve overall health outcomes. I value being a dependable resource—whether that’s simplifying regimens, strengthening treatment plans or helping patients better understand their medications.

I especially enjoy the longitudinal aspect of care, getting to know patients over time and being part of their ongoing health journey.

Outside of work, I spend my “free time” enjoying the beautiful chaos of life with my husband and our four children. Their energy keeps me grounded and reminds me why patient centered care matters.

Jason Miller, PharmD, Ambulatory Care Pharmacist

Jason Miller

I began as a pharmacist intern in the St. Cloud Hospital while attending pharmacy school in 2008. I graduated as a pharmacist in 2011 and spent several years working as a community pharmacist in Clearwater. I then supported community pharmacies throughout the state for another decade before coming back to provide patient care in the CentraCare Neurosciences Pain Medicine. I’m also active in the Minnesota Pharmacists Association, advocating for patient access and supporting the pharmacy profession.

Outside of work, I enjoy spending time with my four daughters, of whom I’m endlessly proud.

Erin Salo, PharmD, Ambulatory Care and Inpatient Pharmacist

Erin Salo

I grew up between Richmond and Eden Valley and graduated from Eden Valley-Watkins High School in 2014. In 2019, I joined the CentraCare St. Cloud Hospital pharmacy team as an intern where I worked until graduating from the University of Minnesota College of Pharmacy – Duluth Campus in 2022. I then joined the CentraCare Paynesville Pharmacy team first as a PGY-1 Pharmacy resident in 2022-2023 and then as a full-time team member. Providing care in rural areas is important to me and it’s great to be practicing in communities not far from where I grew up! My goal is to provide education and care for patients closer to their homes.

Outside of work, I enjoy reading, cooking, crocheting, and spending time with my family.

I grew up in Central Minnesota and earned my Doctor of Pharmacy degree from the University of Minnesota in 2020. I’ve been in my role with CentraCare since 2021 and am grateful to serve the community in which I was raised.

Outside of work, I enjoy time with family and friends, relaxing at the lake and traveling whenever I can.

Amanda Smith

I grew up in rural Northern Minnesota. Having experienced the difficulties of healthcare in rural communities, I wanted to be a part of the change and help rural communities have greater access to healthcare. That desire led me to become a pharmacist and led me to become a PGY1 Pharmacy resident at CentraCare, where we are also committed to making rural life healthier. My goal is to help patients understand their medications and their purpose, so they can feel confident and comfortable in their medication regimen.

In my free time, I enjoy sewing and cross-stitching.

My role involves partnering with patients and providers to ensure safe and effective dosing, monitoring therapy outcomes, and providing education to support optimal treatment and reduce risks.

Annie Wenzel, PharmD, Ambulatory Care Pharmacist

Annie Wenzel

I earned my Doctor of Pharmacy from NDSU in 2020. I began my career working in a community pharmacy setting for two years before deciding to pursue a residency in ambulatory care. I matched with Cash Wise Clinic Pharmacy through the University of Minnesota in 2022 for a PGY1 residency and am so grateful for that opportunity to advance my career! I now work as a Clinical Ambulatory Pharmacist, specializing in anticoagulation management and Medication Therapy Management services.

In my free time, I enjoy hiking, reading, pheasant hunting and a well-made cup of coffee.
