Helping Central Minnesota Residents Overcome Barriers to Health Care

Don’t let the lack of health insurance — or not having good coverage — stop you from seeking medical care when you need it.

Project H.E.A.L. (Health, Education, Access, Link) is an outreach program of CentraCare Coordinated Care Services. Project H.E.A.L. offers free health screenings and basic care at various locations across Central Minnesota for those who have little or no insurance.

The goal of Project H.E.A.L. is to overcome the barriers to health care for the under-served population of our communities. We help people overcome the barriers of transportation, inadequate or lack of insurance, financial resources for co-payments and medications, language and culture. None of these should stand in the way of you and your family receiving the quality medical care you need. See Fiscal Year 2025 Summary.

Monthly Locations

Long Prairie

  • Amish Clinic – CentraCare – Long Prairie West Campus (20 Ninth St. SE.)
    • One clinic per month on third or fourth Saturday at 9 am. Please call 320-732-2141, ext. 26868, for updated dates and times

Melrose

  • Melrose Senior Community Center (225 1st St. NE)
    • Second Thursday evening clinic offered each month from 6-7:30 p.m.

St. Cloud

  • Archangel Recovery and Reintegration Ministry (1516 3rd St N)
    • First Monday each month from 1-3 p.m.
  • Eastside Community Outpost (Salem Lutheran Church, 90 Riverside Dr SE)
    • Every Wednesday clinic offered from 1-4 p.m.
  • Legacy Community Outpost (600 13th St S)
    • Every Thursday (May-November) and every other Thursday (December-April) offered from 9 a.m. to 4 p.m.
  • Lincoln Center (630 Lincoln Ave SE)
    • Fourth Wednesday each month from 11 a.m. to 1 p.m.
  • Mosque Wellness Event (712 17th Ave S)
    • First Friday each month from noon to 3 p.m.
  • Place of Hope – Compassionate Hope Family Center (121 Park Ave N)
    • Second Tuesday offered each month from 3-5 p.m.
  • Place of Hope (511 9th Ave N)
    • Third Tuesday clinic offered each month from 8:30-10:30 a.m.

Events with Spanish Interpreters

Project HEAL also has a partnership with a local organization called Fe y Justicia ("Faith and Justice") that offers free monthly clinics in Long Prairie and St. Cloud with Spanish interpreters for individuals without insurance. For more information on these clinics, please call 320-291-6355.

Long Prairie

  • CentraCare – Long Prairie West Campus (24 Ninth St. SE.)
    • First Thursday of each month from 6-7:30 p.m.

St. Cloud

  • Saint Michael Church (1036 Co. Rd. 4 in St. Cloud)
    • Third Thursday of each month from 2-5 p.m.

The partners for Project H.E.A.L. include CentraCare Coordinated Care Clinic, CentraCare Community Health Improvement, CentraCare Community Paramedics, St. Cloud Hospital Emergency Department, Mid-Minnesota Legal Aid, Place of Hope, Catholic Charities, Lincoln Center/Homeless Helping Homeless, Central Minnesota Parish Nurse Program and local congregations.
