Primary Care

CentraCare delivers primary care services to you in long-term care and assisted living facilities. We work alongside you, your family and your facility to develop a plan of care related to your goals. Our primary care team is board-certified and focused exclusively on caring for older adults.

Transitional Care Unit

If you are not well enough to return home after a hospital stay, you may be transferred to a Transitional Care Unit at a short-stay rehabilitation facility. The CentraCare Senior Transitions team will tailor a treatment plan to fit your special needs until you can go home.
