Our compassionate Senior Transitions team sees patients who are receiving rehabilitation at short-stay rehabilitation facility. The team’s onsite care facilitates quicker recovery times and attention to patients’ health care needs during their recovery.

Senior Transitions Services Include:
- House calls made by a doctor or certified nurse practitioner at your short-stay rehabilitation facility
- Care conveniently delivered to you so don't have to travel to appointments or spend time in waiting rooms
The care team will help identify and coordinate additional services for you when you are transitioning back to your home.
Services are paid for by Medicare and all other insurances. There is no additional charge for visits at your short-stay rehabilitation facility.
