MN State High School League Physical Exam Form
Many sports and activities require a physical exam before a child can participate. The Minnesota State High School League requires an updated physical every three years. These physicals are completed by a family medicine provider or pediatrician to determine whether a child can participate with or without limitations, or if the child should not participate. An exam is important to determine if their may be any pre-existing conditions that could put the child at risk for sport participation.
We recommend you schedule a complete well-child exam with your primary health care clinician. This is also a good time to review and update immunizations before school begins.
Please come to the appointment prepared with a copy of the MN State High School League Physical Exam Form signed by a parent or a guardian. Forms are also available at the clinics.
