CentraCare Health Home Care & Hospice offers Health Care Directive sessions

CentraCare Health and Light the Legacy are offering free advanced health care directive information sessions. Certified facilitators and a notary public will be on hand to provide forms, answer questions and assist in completing directives.

A health care directive is a planning tool used to identify who you want to make health care decisions for you if you aren’t able to communicate. It puts your wishes regarding end-of-life care in writing.

St. Cloud Hospital, Hoppe Auditorium 1:30 – 3:00 p.m. on November 7. Register now.
