There are several financial support options available to CentraCare patients, including:

  • Medicaid / Medical Assistance.
  • Income-based financial assistance.
  • No-interest payment plans.

Medicaid and Medical Assistance

Elevate Patient Financial Solutions (Elevate PFS)

CentraCare has partnered with Elevate Patient Financial Solutions (Elevate PFS) to help patients without medical insurance find resources to help pay for medical bills – for example, Medicaid or Medical Assistance. Income-Based Financial Assistance

CentraCare is committed to providing high-quality health care to the community regardless of ability to pay. As part of our commitment to serving the health needs of people who are uninsured or underinsured, CentraCare offers discounts to those whose incomes are less than 250% of the Federal Poverty Guidelines. (All income-based program applicants will also be screened for the Hospital Department Discount Program. See details below.)

To apply for financial assistance, patients must submit a complete application (including supporting documents) via MyChart, via email (to, in person (at any CentraCare entity) or by mail (to CentraCare, Attn: Patient Financial Services, 1406 6th Ave North, St. Cloud, MN 56303).

Hospital Department Discount Program

All Income-Based Financial Assistance Program applicants will automatically be screened for the Hospital Department Discount Program. This discount only applies to services received in a CentraCare hospital department.

You may be eligible for this discount if you meet the following criteria:

  • You do not have insurance or if you have received services in a hospital department that your insurance determines are not covered.
  • Your yearly household income is below $125,000 and you are not already covered by our financial assistance program (detailed above).

You can complete the Income Verification form linked below to see if you qualify. Return the completed form and supporting documents to CentraCare via email at or via mail to CentraCare, Attn: Patient Financial Services, 1406 6th Avenue North, St. Cloud, MN 56303.

No-Interest Payment Plans

CentraCare offers self-service no-interest payment plans to patients, regardless of their income status. Set up a custom payment plan by accessing your account on and selecting Start a Payment Plan.

Bill Pay

Pay your CentraCare bill securely online. Securely pay bills by credit/debit card, electronic check payment (ACH), Apple Pay or Google Pay, or select a payment plan that works best for you.
