Choosing Your Medicare Plan

The information below is to help you make the best choice for you and your family about Medicare coverage. As of January 2, 2021, CentraCare and Carris Health will not participate in the Humana Medicare Advantage program. If you elect this plan, you can still be cared for by us, but CentraCare will be an out-of-network provider. Your out-of-pocket costs may be higher, depending on services received.

If you prefer a health plan that provides in-network benefits for CentraCare services, you may use the information in the table below to contact the insurance companies directly.
