By choosing to schedule your curbside COVID-19 test online, you are agreeing to the General Consent for Admission.

For VA patients: If you receive benefits through the VA, please schedule your COVID test with the VA. Prior authorization is required from the VA for COVID testing done at CentraCare or Carris.

Retesting Following a COVID Positive Test
Testing for COVID is not recommended for patients without symptoms who have previously tested positive within the past 90 days. If you develop new symptoms within 90 days, please contact your health care provider.

Close Contact Exposure

I had a close contact exposure to someone who tested positive for COVID-19 within the past 10 days. I understand a close contact exposure is defined as:
- Less than 6 feet apart
- For a cumulative 15 minutes or greater within a 24-hour period

At this time, other asymptomatic testing at CentraCare remains paused, meaning that travel and event testing are not available at our curbside, clinic, emergency department or urgent care sites.

If these situations apply to you, we encourage you to utilize an at-home COVID-19 test or find a community testing site.

Symptoms of COVID-19

Symptoms of COVID-19 include:

  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea
