St. Cloud Hospital Transitions of Care Program Honored with Innovation Award

ST. CLOUD, Minn. – St. Cloud Hospital’s Transitions of Care program has been selected as recipient of the 2013 Innovation I-Award by the Center for Nonprofit Excellence and Social Innovation (CNESI) Board of Directors. St. Cloud Hospital was honored at the Sept. 26 Innovation Summit and presented $1,000.

The award recognizes St. Cloud area non-profits employing “Innovation” to meet needs, solve problems, and create new opportunities. The I-Awards recipients provide models for doing business differently to achieve efficiency and high quality services.

“The transition from hospital to home is a complicated, sometimes confusing time for patients and their families,” said Gail Olson, care center director, St. Cloud Hospital Home Care and Hospice. “Adding a Transitions Coach to the patient’s care team who can help the patient navigate this transition and the 30 days after discharge, has decreased readmission rates and improved patient self-management skills. Thank you to CNESI for supporting our efforts to help patients spend more nights of sleep in their own beds instead of the hospital. ”
