If you take multiple medications, it can be confusing. Our Medication Therapy Management team of ambulatory pharmacists can help.

Medication Therapy Management (MTM) is a clinic-based or virtual appointment with a specially trained pharmacist that aims to help you get the most out of your medications and help you to meet your health care goals. The pharmacist will help you understand why you are taking each of your medicines, understand how the medicines work, ensure safety to avoid drug interactions and find the best ways for you to take your medications. The MTM pharmacist also will address any concerns you may have about the cost and management of your medication.

At your appointment, you will meet one-on-one with a licensed MTM pharmacist who will review in-depth all your medications, including prescription medicines, herbal supplements, over-the-counter products and dietary supplements. The MTM pharmacist will work closely with your provider to make any adjustments necessary to help you get the most out of your medications.
