It’s time to leave the hospital, but you aren’t ready to care for yourself at home. That’s what Transitional Care is for — to support patients as they recover from a variety of illnesses and surgeries between the hospital and home.
Transitional Care provides:
- A personalized plan of care that focuses on physical, spiritual and social needs
- Daily skilled care delivered by nurses and/or therapists
- A familiar environment that includes activities to help the patient get back to daily living and independence

Most patients of the program come from a hospital setting and are recovering from illnesses or injuries, including:

- Multiple medical problems
Our team is dedicated to meeting your needs. We work with other professionals who have expertise in short stay care including nursing, occupational, physical and speech therapists, therapeutic recreation, nutrition, psychology and pastoral care.
