Central Minnesota Home Health Services

CentraCare Home Health provides remote patient monitoring to manage your health at home. The monitor is individually programmed for you according to conditions set by your health care provider.

Remote Patient Monitoring may be ideal if you have:

  • Congestive heart failure (CHF)
  • Chronic obstructive pulmonary disease
  • Hypertension
  • Other chronic conditions

Benefits of Remote Patient Monitoring

  • Remote patient monitoring can prevent emergency room visits and hospitalizations. When your vital signs are taken on a daily basis, irregularities can be caught before they become problems.
  • Nurses can be sent to your home and, based on the daily data, work directly with your health care provider to adjust medications and diet.
  • You become more aware of your condition and understand the effects diet, activity and other factors have on your health.
  • Remote patient monitoring helps health care professionals work more efficiently. One nurse can observe the clinical health of numerous patients at a time and effectively direct medical attention to those who need it.

How Does Remote Patient Monitoring Work?

  • In just three minutes each day, the monitor collects a variety of vital signs such as heart rate, blood pressure, oxygen saturation, body weight and temperature.
  • The monitor also gathers information on your fatigue and general wellness levels by asking a variety of customized questions.
    • For example, if you have high blood pressure, you might be asked, “Are you experiencing any dizziness today?” You respond to each question by pressing the “yes” or “no” button on the monitor.
  • The data collected by the monitor is reviewed by our clinicians, who can find and deal with even the slightest abnormalities before they become serious. This helps prevent emergency room visits and hospitalizations.

Dear Friends and Neighbors,

CentraCare care givers have been working around the clock for more than 20 months to care for you, your families and friends during COVID. We are committed to caring for every Minnesotan who needs us, and nothing will prevent us from doing so – even during these never-seen-before times.

The challenge of providing this level of care is that our hospital beds are often full. ERs in all of our hospitals are packed. And our clinical teams are exhausted. Early in the pandemic, our community stepped up in amazing ways to help us. We ask that you again join us in fighting this pandemic together.

How can you help?

  • Please get your COVID vaccines and booster shots. They are proven safe and effective in reducing COVID illness, keeping people out of the hospital, and preventing death.
  • If your situation is not an emergency, please use other care options, including:
  • If this is a medical emergency, call 9-1-1, or visit the ER.

Together, we can do this. Thank you for your support.
