CentraCare – Melrose Care Center provides quality, compassionate care with an emphasis on enhancing each resident’s physical, spiritual, emotional and social well-being. The 75-bed care center is made up of two units: Care Center, our regular skilled nursing unit, and Pine Haven, our special care unit for residents with Alzheimer’s disease, memory loss or other behavioral conditions.

  • 24-hour skilled nursing services provided by our dedicated team of RNs, LPNs, certified nursing assistants who specialize in providing direct care.

  • Memory care unit provides care for persons with Alzheimer’s or dementia
  • Rehabilitation services
  • Transportation services – a 15-passenger, handicapped-accessible van transports residents to various community destinations for shopping, dining or other events

CentraCare care givers have been working around the clock for more than 20 months to care for you, your families and friends during COVID. We are committed to caring for every Minnesotan who needs us, and nothing will prevent us from doing so – even during these never-seen-before times.

The challenge of providing this level of care is that our hospital beds are often full. ERs in all of our hospitals are packed. And our clinical teams are exhausted. Early in the pandemic, our community stepped up in amazing ways to help us. We ask that you again join us in fighting this pandemic together.
