We offer a unique program that provides education on your condition and offers advice on diet, exercise, tobacco cessation and medications so you can self-manage your condition. We also offer an array of communication tools to support your success, such as face-to-face office visits, telephone consultations, video visits and telemonitoring of your heart rhythms from your home.

More than 1,000 people throughout Minnesota receive care through our program each year. A dedicated heart failure team provides personalized treatment, taking the time to get to know you so you can set goals, manage your daily living and begin the path to improvement.

Our program has been recognized nationally by U.S. News & World Report, the American Heart Association’s Get With The Guidelines®–Heart Failure program, Blue Cross and Blue Shield and HealthPartners. Our hospital re-admission rates are 3.2 percent lower than the national average.


We are often one of the nation’s leading enrollers in clinical trials, looking at new technology and medical therapy to improve survival and quality of life.

Advanced Heart Failure Solutions

For people with end-stage heart failure, we provide consultations related to a heart transplant or left ventricular assist device (LVAD), a surgically implanted pump that assists the heart. Our team works closely with the University of Minnesota to provide these advanced treatment options.
