The program helps you set goals and manage symptoms that can affect quality of life, such as depression, stress, fatigue, insomnia and labored breathing.

As part of the program, a specialized clinician will meet one-on-one with you and your family to discuss:

  • Your disease state and its progression
  • Symptom management and advanced treatments
  • Health care directives, action plans and an assigned health care agent
  • Your vision for living your best life
  • Short and long-term goals
  • Choices on hospitalization
  • Available resources
