Health Care Directive & Advance Care Planning

Who will speak for you if you can't speak for yourself?

A health care directive is a planning tool used to identify who you want to make health care decisions for you if you aren’t able to communicate. It puts your wishes regarding end-of-life care in writing. For more information, please ask your nurse for a packet or a brochure on Health Care Directives.

Local organization, Light the Legacy, provides information about Health Care Directives, as well as forms you can download and complete. View this video to learn more.
